Initial visit to establish care $95
Annual Physical/PAP $95

Care credit is accepted for products and services.
*Late cancellation fee: appointments cancelled less than 24 hours of appointment time $20
Hormone Pellets
Male pellets + insertion: $600 and lasts an average of 5-7 months.
Female pellets + insertion: $400 and lasts an average of 3-5 months.
*Deposit of 50% required to schedule pellet insertions.
Weight Loss
Medication coverage varies depending on plans and required criteria. A prior authorization will be completed as part of your visit. If medications are not covered under your plan, cost saving alternatives are available.
Labs
Lab prices reflect self-pay rates. Depending on insurance coverage, theses rates may be more affordable than plans with a large deductible.
