This visit is to establish care. We will review your medical history and answer questions you have on your health and how our practice can help you achieve your optimal level of wellness. Please complete all forms advance; bring any labs or imaging you would like to discuss.
Sick Visit

This visit type is intended for when you are sick and need medical attention. Examples are UTI, cough, flu, sinus infection.
Follow - Up

This is a follow up discussion on previous conditions. Examples are a lab review or a medication follow up. A virtual link will be emailed to you.

What to Expect
New Patients
Your first visit as a new patient is to establish care. Please bring identification, proof of insurance (if applicable), health history, family history, list of current medications and printed copies of recent lab work and imaging results. Prior to treatment, consent and notice of privacy forms will need to be signed, we request completion of intake forms 24 hours prior to visit.
Current Patients
Lab/imaging results, evaluation of current treatment plans, requests for lab/imaging orders, prescribing of controlled substances or new medications, changes in health status, and surgical clearance require a scheduled visit with a medical provider.
Urgent Visits
Accommodations will be made to the schedule to prioritize sick patients being seen. These visits are scheduled for 10 - 15 minutes with the provider and will only cover diagnosis and treatment of the current illness.
Arrival Time
If you are expecting to be greater than 5 minutes late to your appointment, please contact the office to determine if your appointment needs to be rescheduled.
Cancellations
Appointments that are cancelled less than 24 hours before scheduled time are subject to a $20 late cancellation fee.
Communication
Questions related to current medications and treatment plans should be discussed during the visit with your provider. The secure portal via Elation Health can be used for follow up questions, if needed. For concerns regarding medication status and refills, please contact your pharmacy directly. Questions about insurance coverage should be discussed with insurance company.
Insurance Coverage
Any questions regarding products and/or services covered by your insurance company should be directed to your insurance company by calling the number on the back of your card. Prior authorizations are subject to a $20 fee, so it is beneficial to find to know the coverage of your plan.
